  • Record name, address, and phone numbers of any witnesses, e.g., anyone who saw the accident but was not involved in it. 
  • Note the date, time and location of the accident. Record details like cross streets and lane configurations.

  • Always report theft and vandalism losses to the police.

  • Names
  • Driver license number/state
  • Addresses
  • Telephone numbers 
  • Dates of birth and gender
  • Insurance company name and policy information

Seek information from police regarding injured parties names, addresses, dates of birth, gender and extent of injuries.

Regarding Other Vehicles:

  • Owner name and address 
  • Make, model, year 
  • License plate or tag number and state 
  • Damage to other vehicles
